تأثیر تمرینات بازتوانی شناختی و حرکتی با تحریک شنیداری بر کارکرد اجرایی و خودکارآمدی درد زنان مبتلا به ام‌اس

چکیده
مشکلات شناختی اصلی‌ترین عامل در محدودیت فعالیت و مشارکت در افراد مبتلا به ام‌اس است. درمان‌های بازتوانی را می‌توان به‌عنوان روشی مؤثر در بهبود علائم ناشی از بیماری ام‌اس مورد مطالعه قرارداد. مطالعه حاضر باهدف بررسی تأثیر تمرینات بازتوانی شناختی و تحریک شنیداری ریتمیک بر کارکرد‌های اجرایی سرد و خودکارآمدی درد در زنان مبتلا به ام‌اس انجام شد. نمونه آماری پژوهش 30 زن مبتلا به ام‌اس بود، که به صورت هدفمند، بر اساس معیارهای ورود به مطالعه انتخاب شدند، سپس آزمون بررسی مختصر وضعیت شناختی را تکمیل نمودند و به صورت تصادفی به دوگروه تمرینات بازتوانی شناختی، بازتوانی تحریک شنیداری تقسیم شدند. آزمودنی‌های هر دوگروه به مدت 8 هفته، هفته‌ای 3 جلسه 60 دقیقه ای به تمرینات خود پرداختند. در پیش آزمون و پس آزمون، آزمودنی های هر دو گروه پرسشنامه خودکارآمدی درد را تکمیل و آزمون ان بک و آزمون برو/نرو را جهت ارزیابی کارکردهای اجرایی انجام دادند. یافته‌های تحلیل واریانس با اندازه های مکررنشان داد پس از حذف اثر پیش آزمون، در پس آزمون دوگروه در متغیرهای حافظه کاری، خطای ارائه و خطای حذف پیشرفت معناداری کرده اند. تمرینات بازتوانی شناختی به طور معنادار تاثیر بیشتری در بهبود کارکرد های اجرایی سرد و خودکارآمدی درد زنان مبتلا به ام‌اس دارد. پیشنهاد می شود از این تمرینات در توانبخشی افراد مبتلا به ام اس استفاده شود.

Objective: Cognitive impairments are among the primary factors limiting activity and participation in individuals with multiple sclerosis (MS). Rehabilitation interventions are considered an effective approach to alleviating MS-related symptoms. The present study aimed to examine the effects of cognitive rehabilitation training and rhythmic auditory stimulation on cold executive functions and pain self-efficacy in women with MS.
Methods: The study sample comprised 30 women with MS, selected through purposive sampling based on predefined inclusion criteria. Participants completed the Expanded Disability Status Scale (EDSS) and the Mini-Mental State Examination (MMSE) before being randomly assigned to two intervention groups: cognitive rehabilitation training and rhythmic auditory stimulation rehabilitation. Both groups underwent an 8-week intervention, with three 60-minute sessions per week. Pre-test and post-test assessments included the Pain Self-Efficacy Questionnaire, the N-Back working memory task, and the Go/No-Go task.
Results: The results of repeated measures analysis of variance showed that after removing the pre-test effect, in the post-test, the two groups made significant progress in the variables of working memory, presentation error, and omission error.
Conclusion: Cognitive rehabilitation exercises have a significantly greater effect on improving cold executive functions and pain self-efficacy in women with MS. It is suggested that these exercises be used in the rehabilitation of people with MS.
